    An effective massage session involves good communication as well as technique. Communication is critical for a successful outcome.

    19162202 photos comA health questionnaire aids that communication. Even if clients wish to have massage to maintain good health, a health questionnaire/intake form is always a good idea. It will ensure they bring up relevant issues, give you a general picture of their well-being, and alert you to contraindications.

    For new clients, ask them to come in 15-20 minutes prior to their appointment to allow time to fill out the form. Carefully review the completed form, asking for clarification and encouraging them to elaborate as needed. Weigh out any contraindications, referring them to a physician first, if necessary, before providing this service.

    For returning clients, be sure to ask if there have been any changes in their health, and note them accordingly on their form.

    If a client is reluctant to provide this information, explain how massage affects the body and could affect pre-existing conditions. You have the right and responsibility as a massage therapist to request this information prior to providing any service, and this information protects you too. For example, performing lymphatic massage on someone with contraindications puts you at legal risk. (Remember, you always have the right to decline providing your services.)

    Of course, as you have access to privileged information, clients’ health records should always be protected and kept in a locked file. Assure your clients that their information is kept confidential.
